What is Vitiligo?

Vitiligo is a chronic pigmentary disorder characterized by the loss of melanocytes, the cells responsible for producing skin pigment (melanin). This results in well-defined depigmented (white) patches on the skin, which may appear on any part of the body.

Although medically benign and non-contagious, vitiligo is a clinically significant condition with complex underlying biology. Normal skin colour is determined by melanocyte activity within the epidermis. In vitiligo, these melanocytes are selectively destroyed, leading to complete loss of pigment in affected areas. Current evidence supports an autoimmune mechanism, in which the immune system targets and eliminates melanocytes. The result is a sharply demarcated contrast between affected and unaffected skin, often more noticeable in individuals with darker skin phototypes.

Clinical Types of Vitiligo

Vitiligo is classified based on its distribution and pattern of involvement, which has implications for prognosis and treatment response.

  1. Non-Segmental Vitiligo (NSV) - This is the most common form and is typically symmetrical in distribution. Depigmented patches often appear on the face, hands, elbows, knees, and around body orifices. Non-segmental vitiligo tends to follow a chronic, relapsing course, with periods of stability and progression. It is also more commonly associated with autoimmune conditions.
  2. Segmental Vitiligo - Segmental vitiligo presents as unilateral depigmentation, typically confined to one area or dermatome. It often develops earlier in life and tends to stabilize more quickly than non-segmental forms.
  3. Other Clinical Variants - Less common patterns include focal vitiligo (localized patches) and universal vitiligo, where extensive depigmentation affects most of the body surface area. These variants are less frequent but clinically important in determining management strategy.

What Causes Vitiligo?

The exact cause of vitiligo remains multifactorial, with several interacting biological mechanisms:

  • Autoimmune destruction of melanocytes, the primary and most widely accepted mechanism
  • Genetic predisposition, with increased risk in individuals with a family history
  • Oxidative stress, contributing to melanocyte damage and vulnerability
  • Triggering events, such as skin injury (Koebner phenomenon), psychological stress, or illness

Vitiligo is also associated with other autoimmune conditions, reinforcing its systemic immunological basis.

How is Vitiligo Treated?

There is currently no definitive cure for vitiligo. However, modern dermatology offers effective strategies to stabilize disease progression and in many cases, induce repigmentation.

  1. Topical Therapy - For localized disease, topical treatments are often first-line. Corticosteroids and calcineurin inhibitors are used to modulate the immune response and create an environment conducive to melanocyte recovery.
  2. Phototherapy - Narrowband ultraviolet B (NB-UVB) phototherapy is a cornerstone of vitiligo treatment. It works by suppressing local immune activity and stimulating melanocyte regeneration and migration. This modality is particularly effective in widespread or progressive disease.
  3. Systemic Intervention - In rapidly progressive cases, short courses of systemic corticosteroids may be used to stabilise disease activity. Emerging therapies, including targeted immunomodulators, are expanding the treatment landscape for selected patients.
  4. Procedural Approaches - In stable vitiligo, surgical options such as melanocyte transplantation may be considered. These approaches are highly specialised and reserved for carefully selected cases.
  5. Adjunctive Care and Photoprotection - Comprehensive management extends beyond medical treatment. Depigmented skin lacks natural photoprotection, making it highly susceptible to ultraviolet damage. Daily use of broad-spectrum sunscreen is essential to prevent sunburn and reduce contrast between affected and unaffected skin. Cosmetic camouflage may also be used as a supportive measure where appropriate. Equally important is the avoidance of unproven or aggressive treatments, which may trigger further depigmentation through irritation or injury.

Vitiligo is a chronic condition with an unpredictable course. Treatment requires consistency, patience, and realistic expectations, as repigmentation is often gradual and varies between individuals. Early intervention improves the likelihood of stabilisation and response. Ongoing management focuses on maintaining disease control and supporting pigment restoration where possible. With a structured, evidence-based approach, vitiligo can be effectively managed minimising progression and improving overall skin uniformity.
